Costs for the exam are $130. Exams usually take between 30-60 minutes, though this may vary.

For the following conditions, you will need to bring extra paperwork:

  • Sleep Apnea
    • Printout from your sleep apnea machine
  • Diabetes
    • HgA1C Lab result
  • Heart Disease
    • Please check with me ahead of time for what is needed.

If you have a condition that is found during the examination, that might disqualify you from your CDL, I am happy to help you treat the condition.

About CDL Physical Exams or DOT Medical Examinations

Commercial truck drivers and bus drivers are required to pass a physical or medical exam every other year.  You may need to complete the exam yearly if you have a chronic medical condition.  I am a DOT-approved medical examiner.  My L & I registry number is 2711245942.  I was certified 04/17/14.

There are 2 types of certificates and you need to choose which type of certification you need at your visit.

1.  Interstate (Federal)

     This classification means that you drive between states. 

  • from one state to another state or a foreign country, or
  • between two places within a state, but during part of the trip, the CMV crosses into another state or foreign country, or
  • between two places within a state, but the cargo is part of a trip that began or will end in another state or foreign country.

    So you may need this type of certification if you haul materials to the port to be exported.

    You must take your certificate to the Department of Licensing as they will attach your certification to your Driver License.

2.  Intrastate (State)

     This means you will be driving in Washington State only.  (You don't need to take this certificate to the Department of Licensing).
